Consent, Waiver and Liability Release Rediker Pet Transportation Services strives to provide a safe and enjoyable environment for all pets in its care. However, I understand that pet can sometimes be injured or become ill during transportation and understand that every pet reacts differently and that animals, by nature, are unpredictable. Therefore, on behalf of myself (and my personal representatives, heirs, executors, administrators, agents and assigns) I read, understand, and agree to the following: I hereby waive, release, and discharge Rediker Pet Transportation Services and its owners, agents, officers, employees, or volunteers from any and all liabilities that result in injury to, or the loss of my pet, or any other property of mine which arises in any way out of the transportation provided by Rediker Pet Transportation Services. I hereby agree to defend, indemnify, and hold harmless Rediker Pet Transportation Services, its owners, agents, officers, employees, or volunteers from and against any and all claims of any nature including all costs, expenses and attorneys’ fees, which arise in any way out of the transportation provided by Rediker Pet Transportation Services. I understand that Rediker Pet Transportation Services will be providing transportation for my pet. Rediker Pet Transportation Services agrees to exercise reasonable care of my pet during transportation. I acknowledge that allowing Rediker Pet Transportation Services, its owners, agents, officers, employees, or volunteers to transport my pet involves risks of injury to my pet during transportation. I knowingly accept and assume all known and unknown risks in allowing my pet to be transported. I recognize the risk of injury that accompanies transportation of my pet and acknowledge that this release is being relied upon by Rediker Pet Transportation Services to permit transportation of my pet to or any other necessary location, such as an animal hospital or veterinarian clinic. I accept any and all conditions, rules, and regulations declared by Rediker Pet Transportation Services, associated with the activities, use of the Rediker Pet Transportation Services facilities and transportation, and hereby agree to comply with them. Rediker Pet Transportation Services will take precautions to prevent pet from running away from their vehicles or property but pet’ behavior cannot be perfectly controlled at all times. I will not hold Rediker Pet Transportation Services responsible if my pet finds a way to escape during care or during transportation. Rediker Pet Transportation Services may, at any time, remove a pet from transport, or refuse a pet transportation, if the pet exhibits behavior that could be harmful to the pet, other pet, or staff, such as:  Aggression or overly dominant behavior;  High anxiety or fear;  Refusal of pet to load into vehicle;  A condition or behavior rendering pet generally not fit for transportation;  If, in the opinion of Rediker Pet Transportation Services employee or agent, transportation is not in the best interest of pet. I understand that pet can sometimes be injured or become ill during transportation. I accept that Rediker Pet Transportation Services is not in any way responsible for any injury, stress or illness my pet may suffer while under Rediker Pet Transportation Services’s care or while being transported by Rediker Pet Transportation Services. I will not hold Rediker Pet Transportation Services liable, financially or legally, for any physical, mental, or health issues that arise during or because of my pet’s transportation by Rediker Pet Transportation Services. This includes, but is not limited to: pet bites, minor cuts and scratches, bone breakage, tissue damage, joint problems, stress, or diarrhea.
